Registration is required for all attendees. To register, complete and return the registration form to Kristen Burt at the Office of Regulatory Affairs either by mail: 107A Olds Hall, Michigan State University, East Lansing, MI 48824, fax: 517-353-2976 or as an email attachment to IRBEducation@ores.msu.edu. An email confirmation will be sent when your registration is processed and a receipt will be emailed once payment (if applicable) has been processed.
The registration fee is waived for MSU and CRIRB affiliated individuals. For non-MSU or non-CRIRB affiliated individuals, the registration fee is $50.00. This fee includes materials and afternoon refreshments. Lunch is not included.
The registration deadline is March 22, 2006 . Please register early, as seating is limited. If you register and are unable to attend, please notify Kristen Burt by phone: 517-432-4500 or email: IRBEducation@ores.msu.edu.
